Your voice matters! I’m excited to announce that the 2025 Community Health Needs Assessment (CHNA) has been launched and we need your input on the community survey. This survey plays a crucial role in guiding the CHNA for nonprofit hospital systems (required every three years) and for local health departments (required every 5 years).
What is a CHNA?
A CHNA gathers essential data on health behaviors, social and environmental factors, and community input. By analyzing this information, we can identify trends, gaps, and disparities affecting our residents. These insights help drive health investments and guide the development of new programs to better serve our community.
Who is conducting the community survey?
The Community Health Partnership of Marinette & Menominee Counties is leading this effort. The group consists of eight community organizations: Marinette County Public Health, Aurora Medical Center-Bay Area, Provident Health Foundation, Emplify by Belling Health, NorthLakes Community Clinic, Public Health of Delta & Menominee Counties, M&M Area Community Foundation, and the Superior Health Foundation. By collaborating on a single survey, we maximize participation, reduce survey fatigue, and foster stronger, more coordinated solutions.
How is community survey data used?
The findings will be shared with the community and used to:
-Shape the community health implementation strategies of the hospital systems and implementation plans for the Marinette and Menominee County Health Departments
-Support local organizations in developing their own strategies
-Strengthen grant applications for local programs
-Provide education and training opportunities to benefit the community
How can you help?
The survey is open now through June 18 for Marinette and Menominee County residents ages 18 and older. It only takes approximately 5 minutes to complete and is available in English and Spanish.

-Share the survey link through your newsletters, emails, and social media.
-Bring up the survey at meetings and community events to encourage participation.
-Post about the survey on your social media platforms.
